Certificates of Suitability (COS), the COS Holder must submit a COS application form with the
updated premises name for consideration by Council.
COS application forms are available here.
The form notes other requirements on it such as a map and floor plan, but these are not required
when changing a premises name. There is also no fee associated with a premises name change.
PAS Regulations 38 – 43 outline the requirements for veterinary premises names. Please read these
carefully before applying to update a premises name. Please also note that in accordance with
Section 54 (2) of the Veterinary Practice Act 2005, as amended, veterinary premises names, as it
appears on the COS Register, cannot include the term ‘Limited/LTD’. The completed form can be
returned by post to the VCI office or a scanned copy by email to pas@vci.ie.
The premises name change will then go before the Practice Premises Committee (PPC) for
consideration, followed by the Council for a final decision at the next possible round of meetings. For

If the premises name change is granted by the Council, a new Certificate of Suitability number and
certificate will issue by post to the premises and the COS Register will be updated.
