When a change in Certificate of Suitability (COS) Holder is required at a registered veterinary
premises, the proposed new COS Holder must submit a completed COS application form in respect
of the premises.

The form notes other requirements on it such as a map and floor plan, but these are
not required when changing COS Holder. There is also no fee associated with a change in COS
holder. COS Application forms are available here.

The premises current COS holder must submit a completed COS Revocation Request form confirming
the proposed change. The COS Revocation Request form is available here.

The change in COS holder will then go before the Practice Premises Committee (PPC) for
consideration, followed by the Council for a final decision at the next possible round of meetings. For

The completed forms can be returned by post to the VCI office or a scanned copy by email to
pas@vci.ie.

If the change in COS Holder is granted, a new Certificate of Suitability number and certificate will
issue by post to the premises and the COS Register will be updated.
